All existing clients that are requesting “proof of service” documentation including, but not limited to letters related to school accommodations, service animal accommodation letters, court ordered paperwork, etc. will be subject to a fee of: $200 per hour to complete documentation $1,500 per court appearance should you be court ordered to treatment and I am subpoenaed on your behalf
